2011 Surveys

The Connecticut After School Network is collecting information from youth, parents and providers to inform statewide policy on after school and summer programs. Please complete the appropriate survey by clicking on the link below:

Youth Survey (This on-line survey branches to different questions for those age 14 and older, and other questions for those 13 and younger)

Provider Survey

We are expecting to conclude the survey on January 3, 2012. Completed paper surveys should be mailed before then to the Connecticut After School Network, 12 Melrose Avenue, Branford, CT 06405.
